STEP 2
If you’ve been diagnosed by a doctor with a condition that makes you eligible to receive a medical marijuana card, such as any condition listed in our Eligibility Section, you need to upload this along with your driver’s license.
Accepted file formats are: jpg, gif, png.
What counts as a “Proof of Condition?”
If you have been diagnosed with a condition that you require medical marijuana for, such as any condition listed in our Eligibility section, you should be able to get a record of this from your doctor. This counts as your “Proof of Condition”.
Proof can also be:
- A diagnostic code (received from your doctor)
- A copy of a prescription
- A screenshot of diagnosis or prescription from your patient portal
- A photograph of your medication bottle(s) showing the medication name, prescribing doctor and your name
- An MRI or an X-ray

THE FINE PRINT
IMPORTANT TO KNOW
You are required to have a qualifying condition to be approved for medical marijuana. Medical marijuana doctors cannot diagnose and recommend patients for medical marijuana.
CANCELLATION POLICY
We are committed to providing exceptional care. Unfortunately, when one patient cancels without giving enough notice, they prevent another patient from being seen. To avoid a $30 cancellation fee, just message us at 813.774.3474 before 10:00 am the day prior to your appointment or the Friday before if your appointment is on Monday.
DISCLAIMER
By submitting and signing our Patient Intake forms you acknowledge you are a patient of Medical Marijuana Clinic, not a patient of the physician you will see at your scheduled appointment. If at any time that physician is no longer associated with Medical Marijuana Clinic, you will be activated in the Medical Marijuana Clinic’s medical director’s registry account.
PATIENT’S TRANSFERRING FROM OTHER CLINICS
If you are registered in the MMUR and transferring your care to our clinic, when scheduling your appointment, select one of the locations listed as ‘Recertification’, then select ‘Transfer’ service. You are not required to provide the Proof of Condition but will need to upload a photo of your medical marijuana card instead.
Before our office can enter your new certification and orders, you must deactivate your registry account from your previous physician in the MMU Registry account. Follow this link for instructions.

MEDICAL MARIJUANA CARD ELIGIBILITY
If you are a Florida resident and have one of the qualifying conditions listed below, it is likely that you are eligible for a medical marijuana card.
You Are a Permanent or Seasonal Florida Resident
To be eligible for a medical marijuana card, you must either live in Florida permanently or are someone who:
- Resides in Florida for a period of 31 consecutive days in each calendar year
- Maintain a temporary residence in Florida
- Returns to the state, country, or jurisdiction of his or her residence at least one time during each calendar year
- Is registered to vote or pays income tax in another state or jurisdiction.

The Process is super easy. Most patients say after their appointment, “Is that it? If I would have known it was that easy, I would have done it much sooner!”
- Complete the easy online Patient Intake forms in Step 1.
- Upload your Proof of Condition and drivers license in Step 2.
- Schedule your appointment online in Step 3 with one of our certified medical marijuana doctors.
You will receive your medical marijuana card approval after you submit your application to the State. It only takes about 2-3 business days. For more details, keep reading!

Once you are approved by the doctor, here are the next steps:
- The doctor registers the patient in the Medical Marijuana Use Registry (MMUR).
- The State emails login credentials to you.
- You complete the application & submit the $75 registration/card fee.
- An approval email is sent within a few business days.
- You can then make a purchase at the dispensary by showing your Florida drivers license or Florida ID – just make sure your orders have been entered into the registry before you head to the dispensary.
- The State will mail your medical marijuana card within a few weeks of receiving your approval email.
You Might Be Thinking...
Why only a 7-month period?
Florida law requires a new certification and orders every 210 days, this means you must get re-certified every 7 months. But don’t worry – all you must do is schedule your recertification appointment online.
Why are medical marijuana cards good for 12-months but my certification is only good for 7-months?
We aren’t really sure why the State set it up like that, but they probably had a good reason. We will remind you in about 6-months from your doctor’s appointment that it is time to recertify. The State will send you a reminder to renew your card. Be sure to keep an eye out for those email reminders!
Can I just visit my regular doctor, do I still need to use your service?
Florida law also requires you to be seen by a certified medical marijuana doctor to obtain their approval for certification and medical marijuana orders. Once you have proof of condition from your doctor, follow our easy 3-step process to receive your medical marijuana card as quickly as possible.
